Welcome to SMILE Psychiatry and Behavioral Health! We are committed to providing high quality care and increasing access to mental health treatment.
We are contracted with a select number of insurance companies. However, this is not a guarantee of payment of services. We strongly encourage you to contact your insurance company to verify that your provider is in your network, that services will be covered, and to understand your deductible and/or copayments. You are ultimately responsible for charges in the event that insurance does not cover the fees charged by the clinic.
The rates listed on our website are for your convenience and represent an estimate only. Specific billing codes used for each visit will be in line with best billing policies and procedures. Your insurance has contracted or discounted rates for each of the charged billing codes and you may contact them for more information and pricing. This information is used to give you an estimate for typical services provided. For questions or with any concerns, please contact our office at 801-406-6256.
If you choose to pay for services up front with a cash, credit card, or HSA card, we do offer a “Prompt Pay” cash discounted rate when a credit card is on file and services are paid for within 48 hours of your appointment (this only applies to those paying with cash and not billing insurance for their visits).

Billing codes, charges, cash discount, and average insurance rates
Financial Policies and Fees
- An interest rate of 6% will accrue on your account for any unpaid balance after 60 days
- Accounts with no payments received after 120 days will be turned over to a third party collection agency.
- Copays/Coinsurance/Deductibles/Self Pay rates are due at time of service.
- Credit Cards are kept on file and will be run within 48 hours of your appointment, no show, late cancellation, or when patient ammounts due are received from insurance companies.
